A BREVIN community psychiatric nurse will assess you at home to help identify your clinical needs. Care plans are developed jointly with you and the most suitable specialist responsible for delivering them is identified.

BREVIN is a Community Mental Health Team (CMHT), including community psychiatric nurses and psychological wellbeing practitioners supported by occupational therapists, psychologists and consultant psychiatrists.
Care plans are delivered via regular home visits by a community psychiatric nurse or a psychological wellbeing practitioner, who can also live with you for a period of time if necessary. BREVIN will co-ordinate with other professionals, including you current psychiatrist, therapist or GP’s where indicated in your care plan.
